Why isn’t my doctor in-network?
We were unable to reach agreement with UnitedHealthcare of Arizona on new provider agreements for our practices in Arizona. We will continue to work with UnitedHealthcare to implement new contracts for our Arizona practices and rejoin the UnitedHealthcare provider network.
Our practices continue to participate with other health insurance plans in Arizona, including Blue Cross Blue Shield, Aetna, Cigna and Health Net to name a few.
Now that my physician is out-of-network with UnitedHealthcare, can I still see my physician?
Since our practices in Arizona will not be participating in the UnitedHealthcare of Arizona provider network as of July 1, 2015, you can take the following steps to schedule an appointment with your physician:
Contact UnitedHealthcare of Arizona at 800-985-2356 directly and request to speak with the Member Services department. You will need to tell them why it is important for you to continue to see your current doctor of choice. You will also need to request an authorization for your out-of-network doctor if an authorization is required under your UnitedHealthcare benefit package.
Contact your primary care physician and have them call UnitedHealthcare of Arizona at 800-985-2356 and request to speak with the Provider Relations department. Your primary care physician can request an out-of-network authorization on your behalf, if an authorization is required under your UnitedHealthcare benefit package.
What if I cannot see my doctor on/or after July 1?
We encourage you to contact your employer and/or UnitedHealthcare of Arizona at 800-985-2356 and request to speak with the Member Services department. You will need to tell them why it is important for you to continue to see your current doctor of choice. You will also need to request an out-of-network authorization at that time if an authorization is required under your UnitedHealthcare benefit package.
How much will it cost me to see my doctor on an out-of-network basis?
We can contact UnitedHealthcare of Arizona for you and provide you with an estimate of what your out-of-pocket expenses will be for out-of-network services prior to your appointment.
If you have out-of-network benefits and/or have obtained an out-of-network authorization, UnitedHealthcare of Arizona will process the claims for your service and apply their own internal schedule of a payment amount for the service.
If you do not have out-of-network benefits and UnitedHealthcare of Arizona is unwilling to authorize the out-of-network services, the total charge for your service will be denied by UnitedHealthcare of Arizona and you may be subject to a higher total expense. In such instance, we have several payment options available, including prompt payment discounts and payment arrangements.
Can you recommend another provider for me to see?
If approved for an out-of-network authorization, you will still be able to be seen by your physician. Otherwise, you will need to contact the Member Services department at UnitedHealthcare of Arizona at 1-800-985-2356 to ask for a listing of participating doctors in their network.
Please note that the Pediatrix offices located at 15650 N. Black Canyon Hwy, Phoenix, AZ 85053 and 2030 West Whispering Wind Drive, Phoenix, AZ 85130 are not affiliated with Obstetrix Medical Group of Phoenix/Phoenix Perinatal Associates, Arizona Pediatric Cardiology, Obstetrix Medical Group of Arizona/Tucson Perinatal Services, Neonatal Specialists or NAL and are not part of this agreement with UnitedHealthcare of Arizona.
